Surgical Fees and Payment

Payment for services rendered is due at the time the services are provided, unless other arrangements have been made in advance. Feel free to contact our office to discuss financial arrangements. Payments can be made by cash, personal checks and money order. We also accept Visa, MasterCard, Discover credit cards or Citihealth Card.

We will provide you with an estimate of the fees for your care. In order to complete an estimate, we must have a written referral from your dentist or physician, and in most cases, current diagnostic X-rays.

If any questions or problems arise regarding your account, please call. Many times a simple telephone call will avert misunderstandings.

Insurance Policy

All insurance cases require a prepayment of the deductible, co-payment and any other non-covered benefits. In most cases, insurance does not cover the full cost of surgical care. It is designed to reduce your cost, but not to eliminate it completely. Our staff will help you receive the maximum benefits available under your policy. However, please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. If you have questions regarding your specific coverage, you can contact your insurance company or the human resources department at your employer. We can provide diagnostic and procedure codes and fee estimates for your planned care.

Siouxland Oral & Maxillofacial Surgery Associates participates with Avera Health Plan, Blue Cross Blue Shield, Dakota Care, Delta Dental of SD, Delta USA, Federal Blue Cross Blue Shield - Dental, Medicare, Sanford Health Plan, SD Medicaid, Sioux Valley Health Plans. We do not participate with any other insurances, HMO or preferred provider organizations.
