In some cases, an initial exam or consultation will be necessary prior to scheduling surgery with one of our doctors. This initial visit, your diagnosis and treatment options will be explained. We will answer any questions you might have about a planned procedure and anesthetic.

A pre-operative consultation and examination is required for patients undergoing IV anesthesia for surgery. For most surgeries, the pre-operative evaluation and surgery will be completed at the same appointment. This will require some information gathering and preparation prior to your appointment. We ask you to help us by providing us with the following information:
  • Your surgical referral slip and current X-rays.
  • If you have medical or dental insurance, bring the necessary completed forms. This will assist us in filing your claim.
  • A list of the prescription and non-prescription medicines you are currently taking.
  • Please let us know of any medical condition for which you are being treated (such as high blood pressure, diabetes, artifical heart valves, heart murmur, rheumatic fever, joint replacement, steroids or blood thinners).
  • Please inform us of any physical disabilities so that we can make your visit to our office more comfortable.
IMPORTANT: A parent or guardian must accompany all patients under the age of 18. Legal guardians need to provide documentation of their status.

X-Ray
If your dentist or physician has taken X-rays, please ask them to forward them to our office. You will then receive a telephone call to schedule an appointment for your surgical care.
